( canon ) Star Wars: the Clone Wars
( summary ) Here
( canon point ) Defaulting to after S07E04, but before Order 66. Can play from any point in the series.
( notes ) Familiar with prequels, Clone Wars, Kenobi series and, faintly, with the original trilogy. Haven't seen Rebels or Bad Batch (yet), nor the Mandalorian or most of the sequels + Rogue One and Solo. Not familiar with anything in the EU or comics beyond fandom mentions.

( physical affection ) Go for it. Unless you're one of his brothers (in arms), however, he might not appreciate it.
( physical violence ) Sure, but he's a highly trained soldier who's efficient and deadly both in hand to hand combat and on the battlefield.
( relationships ) F/M or M/M. Outside memes that are specifically designed for it, I prefer not to assume any romantic CR unless relevant canon mates. No shipping with his fellow clones.
( psychic info ) Ask. Quick overview, he suffers nightmares of battle and fighting at night, connected to the chip in his head.
( magical info ) No magical powers.
( medical info ) In extremely good health. Depending on canon point, he has a chip in his head that will aggrevate him towards the Jedi when activated.
( offensive subjects/triggers ) Rex is a soldier and comes from a heavily war-reliant context, so there will be references to combat and killing. Let me know if this makes you uncomfortable and I'll steer clear.
